DEPARTMENT OF STATE HEALTH SERVICES Case Manager (Social Worker) in HARLINGEN, Texas

Job/Position DescriptionThe Case Manager (Social Worker II) performs complex social services work under the Program Supervisor IV. This position works under minimum supervision with considerable latitude for the use of initiative and independent judgment.The Case Manager position provides comprehensive case management consultation for families with children who have health risks, conditions or special health care needs. Duties include overseeing case management services including traveling to conduct home visits, completing intakes, assessments, service plans, and follow-ups on caseload, linking families to community resources, conducting presentations and communicating program services and/or changes in a positive and accurate manner. Additionally, this position actively participates and advocates in Community Resource Coordination Groups (CRCG), case management coalitions, community organizations, and works collaboratively with public health staff in the event of a regional or state disaster.This position will travel up to 50% of the time and will be Headquartered in Harlingen, Texas with travel to Brownsville, Texas. The Case Manager must be Bilingual (English and Spanish) as they will have regular interactions with individuals who have limited knowledge of the English language.Essential Job Functions(45%) Case ManagementProvides specialized social work consultation and comprehensive case management services to families including conducting home visits for comprehensive assessment of home environment and needs, Family Needs Assessment, Service Plan, and follow-up services. Provides information and referral services, linkage to community resources, family support, advocacy, and service coordination. Ability to communicate in English and Spanish while working with families with limited English proficiency.(20%) Technical Assistance/Quality Assurance/TrainingsProvides consultation, presentations, and training to agencies for programs and assists in training new staff, volunteers and social work interns. Provides program presentations to public to promote awareness of services available to children with health risks, conditions or special health care needs.(20%) Community Organization/Resource DevelopmentActively participates in community organization functions and serves as the agency representative to collaborate with other groups and community entities on health, social work, case management and other related issues.(15%) Professional and Team DevelopmentAttends trainings and conferences to meet licensing requirements and continues to develop professional knowledge and skills relevant to job duties.
