As a Family Nurse Practitioner, you'll provide comprehensive healthcare services, including physical and psychological exams, basic healthcare treatments, and patient education. Key Responsibilities:

  • Conduct physical or psychological exams and record patient histories.

  • Provide basic healthcare treatment.

  • Write prescriptions and orders for medical/psychological tests.

  • Review patient conditions and determine appropriate courses of treatment.

  • Attend to patient needs with patience, care, compassion, and understanding.

  • Function independently to perform age-appropriate history and physical exams for complex, critical, and chronically ill patients.

  • Order and interpret diagnostic and therapeutic tests relative to patient’s age-specific needs.

  • Prescribe appropriate pharmacologic and non-pharmacologic treatment modalities.

  • Implement interventions to support patient stability, including monitoring the effectiveness of interventions.

  • Facilitate Medication Services within Substance Use Disorder programs as applicable:

  • Assess patient conditions and conduct mental status exams.

  • Review symptoms and screen for medication side effects.

  • Review participant functioning and assess patient’s ability to self-administer medications.

  • Provide education regarding the effects of medication and its relationship to the patient’s substance use disorder and/or mental health disorder.

  • Stay abreast of best practices for the treatment of addiction and mental health disorders.

  • Adhere to service delivery expectations.

  • Facilitate patient transitions within the healthcare setting by making appropriate referrals and collaborating with multidisciplinary team members.

  • Oversee care coordination activities, including daily huddles, obtaining test and referral results, and communication with outside facilities and providers.

  • Develop and approve standing orders as permitted by State law.

  • Determine and direct evidence-based approaches to self-management support.

  • Collaborate with other team members to address the needs of vulnerable populations identified.

Knowledge, Skills, and Abilities:

  • Prefer previous experience providing care at a community health center or similar environment.

  • Current Missouri license in good standing.

  • Obtain a Federal DEA license.

  • Ability to relate well with patients and other healthcare personnel.

  • Social and cultural sensitivity appropriate to an ethnically and economically diverse patient and associate base.

  • Willingness to explore/obtain DATA 2000 Waiver.

Experience and Education Qualifications:

  • Graduate of an accredited school of nursing and accredited Nurse Practitioner Program.

  • Licensed as a RN in the State of Missouri.

  • Licensed as an APRN by the State of Missouri.

  • Two years of experience as a Nurse Practitioner.

Additional Requirements:

  • Willingness to become licensed in additional states, other than the state currently licensed in.

  • Comfort with providing telehealth services to the individuals we serve.
