Upper Great Lakes Family Health Center Outpatient Clinical Therapist (LMSW, LLMSW, or LPC) - Pediatrics in Hancock, Michigan
Upper Great Lakes Family Health is seeking an *Outpatient Clinical Therapist (LMSW) *to serve as a integral member of our care team and serving the patients of the Pediatrics Clinic.

*Responsibilities *
- Provides a full array of behavioral health services to patients from children and adults, including but not limited to, and all within an integrated primary care clinic environment or via telehealth:
- screening and assessment
- care planning and coordination.
- interventions providing brief, focused prevention, treatment and recovery services.
- support with persistent illness.
- additional substance use disorder-related services may be provided if Practitioner has CAADC or MCBAP credentials.
- Experience maintaining confidential information and adherence to HIPAA and CFR 42 Part 2.
This position is a 1.0 FTE (scheduled 80 hours per pay period) located within our Pediatrics clinic.
