Summary This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Responsibilities Total-Rewards-of-an-Allied-Health-VA-Career-Brochure.pdf General The outpatient clinical pharmacist supports the general operations of Pharmacy including prescription/order processing, outpatient dispensing, patient consultation, and perform all pharmacy related activities as assigned and on any shift. The pharmacist provides appropriate selection of drug therapy based upon the pharmaceutical principles of pharmacokinetics and pharmacodynamics; monitoring for efficacy, side effects and clinical outcome; and advises prescribers as appropriate. Provides patient-specific therapeutic drug monitoring and communicates relevant findings and/or recommendations to other health care providers in charge of the patient both orally and/or in writing. Monitoring will include perspective review and intervention in: Therapeutic appropriateness of a patient's drug regimen Therapeutic duplication in the patient's drug regimen Appropriateness of the route and method of administration Degree of patient compliance with the prescribed drug regimen Drug-drug, drug-food, drug-laboratory, drug disease interactions, allergies, appropriateness of drug and dose within the context of the individual patients' medical conditions. Clinical and pharmacokinetic laboratory data to evaluate the efficacy of drug therapy and to anticipate side effects, toxicity, or adverse effects Physical signs and clinical symptoms relevant to the patient's drug therapy Maintains records as required by law. Serves as a drug information resource by providing up-to-date drug information to prescribers, other health care professionals, patients, and caregivers. Reports Adverse Drug Events (ADE), near misses, and medication errors and alignment with VA ADERS reporting program. Recommends formulary alternatives and completes non-formulary and restricted medication consult as assigned. Manages various actions, as appropriate, for medication orders to include flagging prescriptions, managing view alerts, and generating unassigned discontinued orders using service reject. Interpret various VA, Federal (FDA, DEA), other regulatory agency compliance standards (e.g., Joint Commission) Facility and Pharmacy Service policies and be able to apply the same to given situations. Ability to exercise sound judgment in a variety of situations and be able to refer matters of policy making to a supervisor. Supervises and directs pharmacy support personnel in the absence of a supervisor. This includes verification of the daily activities of pharmacy technicians. Works independently with minimal supervision. Ensures pharmacy services are provided in a timely and efficient manner. Keeps pharmacy area and equipment clean, neat, and well-organized. Able to multi-task e.g., able to continue to process orders and answer questions from technical staff and/or patients (via phone or in-person). Trains professional staff including pharmacy, medical staff (including fellows and residents) and students. Interfaces with other healthcare providers and administrators to ensure compliance with established pharmacy and medical center policies and procedures. Responsible for resolving a variety of questions posed by physicians, nurses and co-workers related to drugs, pharmacy services and departmental policies and procedures. Assists with the development of quality and safety improvement initiatives and the collection and analysis of data. Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. Grade Determinations: GS-12 Clinical Pharmacist (Full Performance Level) Experience or Education: In addition to the basic requirements, candidates must meet one of the following: 1 year of experience equivalent to the next lower grade level, or Completion of an ACPE-accredited Pharm.D. program. AND, Pharmacists assigned to this position must demonstrate the following knowledge, skills and abilities (KSAs): Knowledge of professional pharmacy practice. Ability to communicate orally and in writing to both patients and health care staff. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. The actual grade at which an applicant may be selected for this vacancy is GS-12. Physical Requirements: The work involves long periods of moving, standing and pushing carts about the work unit. The work requires regular and recurring bending, stretching, and lifting. The incumbent must be a mature, sensible individual capable of working effectively in stressful situations, demonstrating cheerfulness, compassion, courtesy and concern for staff and patients. Heavy lifting (45 lbs. and over), moderate lifting and carrying (15-44 lbs.), and light lifting (under 15 lbs.), reaching above shoulder, use of fingers, use of both hands, sitting, walking, and standing. Near vision correctable at 13" to 16" to Jaeger 1 to 4, Far vision correctable in one eye to 20/20 and to 20/40 in the other, depth perception. Ability to distinguish basic colors, ability to distinguish shades of colors, hearing (hearing aid permitted), and mental and emotional stability. Environmental factors include, constant noise, dust, working closely with others, or working alone, and protracted or irregular hours of work.
