DEPARTMENT: Social Services SUPERVISED BY: CEO SUPERVISES: None STATUS: Non-Exempt Job summary: In this role you will demonstrate compassionate and resourceful social services skills in a rural healthcare setting. The SCC will provide medically-related social services to patients/residents and their families to attain or maintain the highest level of physical, mental, and psychosocial well-being of each patient/resident. This role includes conducting assessments, coordinating community resources, facilitating care plans, and advocating for patient/resident well-being in collaboration with an interdisciplinary team.
