Essential Job Functions

  • Ensure patient assessment visits are scheduled and performed timely

  • Prepare weekly clinician schedules. Reviews clinician schedules daily or more frequently as needed to determine capacity opportunities and ensure productivity standards are being met.

  • Ensures clinicians are assigned and scheduled in most efficient geography, maximizing clinician efficiency,

matching skill with required care, and optimizing clinician capacity.

  • Ensures missed visits are scheduled by the end of the current day/ week or referred to the Clinical Manager for resolution.

  • Works collaboratively with the field clinicians to ensure all patients are scheduled and completed as ordered.

  • Manage calls from patients and field staff related to scheduling issues. Refers clinical and performance related issue to the Clinical Manager.

  • Completes requested schedules ensuring all order and services (e.g. lab draw, supervisory visits, addon visits or disciplines etc.,) are included as required.

  • Supports Clinical Managers and works collaboratively to ensure timely communication with patients, clinicians, and other office personnel.

  • Runs all applicable reports and responds to work flow taking appropriate actions.

  • Assists with internal transfer of patients between Home Health and Hospice for scheduling.

  • Make all scheduling changes and updates within scheduling system.

  • Schedule changes are communicated to staff affected by the change.

  • Extract referrals from software and following scheduling assignment guidelines.

  • Print various reports for supervisors (expiring cert list, active patients, case manager, etc.)

  • Work closely with Supervisors on staffing and scheduling issues.

  • Consult with supervisor if patients must be moved between staff members.

  • Ensure messages reach the appropriate individuals in a timely manner.

  • Treat all callers with courtesy and respect.

  • Facilitate interpersonal communication on a routine basis.

  • Interact with external customers in a courteous and respectful manner.

  • Interact with coworkers and internal customers in a respectful and professional manner.

  • Maintain accountability for actions taken.

  • Perform effectively in the role of preceptor/mentor to new hires and/or students.

  • Process telephone calls in a positive, courteous and efficient manner.

  • Route calls and messages to appropriate personnel.

  • Cognitive ability to function and concentrate on the essential and specialty functions of the job at a high level.

  • Achieves thoroughness and accuracy when accomplishing an essential or specialty function of the job.

  • Ability to think and concentrate for an entire shift.

  • Other duties as assigned.

Required Qualifications

  • High School Diploma, GED, or Higher level of education.

  • Strong leadership ability, good organizational skills, independent and critical thinking skills, and sound judgment.

  • Strong ability to communicate complex and/or controversial topics and concepts to a wide and diverse audience.

Preferred Qualifications

  • Associates or Secondary diploma program in a secretarial coursework or 1-2 years secretarial experience in a healthcare environment.

License, Certification & Clearances

  • PA Act 34 Pennsylvania Access to Criminal History Clearance.

Supervisory Responsibilities

  • This position has no direct supervisory responsibilities, but does serve as a coach and mentor for other positions in the department.

Position Type/Expected Hours of Work

  • Incumbent will be scheduled based on operational need (rotate shifts, standby, on-call, etc.).

  • Travel may be expected locally between Excela Health locations.
