THE SALVATION ARMY Housing Support Specialist in GREEN BAY, Wisconsin

Interview and ascertain the needs of applicants seeking housing assistance and determine program eligibility. Provide ongoing case management to clients by developing and monitoring individualized case plans designed to help clients achieve the highest level of self-sufficiency as they move through the Continuum of Care. Provides follow-up case management services for a period of at least one year. Identify and utilize additional community resources that will help clients. Refer clients to these services. Record all data in electronic client management database. Maintain financial guidelines when providing assistance in order to stay within budgeted allotments.
