Program Overview Intensive Youth and Family The Youth and Family Intensive Services program consists of several programs that support youth who are at risk of hospitalizations, legal involvement, expulsion from school for a variety of reasons, at risk out of placement, and can benefit from the additional support of individual therapy, group therapy, peer support, and case management services to increase skills, strengthen bonds and reduce the need for outside stakeholder interventions.

This position will support the 180 and Day Reporting Center (DRC) program's youth and families by providing linkage to outside agencies to help families in need, support the youth in activities related to transitioning into adulthood, assisting youth and families in using skills in the home through behavior coaching, working with DRC staff in groups and other activities to help families and their youth remain in the home.

Salary Placements will vary and may be above the starting wage based on factors including but not limited to location, experience, education, and licensure. Salary placements will generally occur between the starting wage and the midpoint of the salary range.

Employees may be eligible for additional pay based on program, evening, night, weekend, or holiday work. Bilingual pay may be available upon successful passing of language validation testing.

Case Managers collaborate with clinical teams to secure resources and services to provide resources and services for clients with mild to severe mental illness and/or substance use disorder challenges. Case Managers work directly with clients to provide education and skill development to help move them toward independence.

Position Summary: With a Collaborative, Customer First, and Compassionate approach, Case Managers will: Embrace the North Range Behavioral Health Mission Statement - Provide compassionate, comprehensive care for people who face mental health and addiction challenges. Collaborate with therapists and other relevant staff to coordinate services, and address client needs and behaviors; prevent deterioration and/or hospitalization through consistency and adequate monitoring as indicated by program scope. Refer, coordinate, and assist clients to access a wide variety of services (e.g., vocational, medical, social security, housing, etc.) based on need and client desire; aid clients in application processes and provide supporting documentation as needed. Offer support, assistance, consultation, and education to relevant providers, family members/caregivers, and other parties when necessary to support clients and minimize barriers. Assist clients in improving their well-being and quality of life by coaching in behavioral skills; educate clients to make informed choices; develop coping skills of clients; help clients to problem solve and negotiate tasks of living daily.

Minimum Qualifications: A Bachelor's degree in human services area from an accredited college or university is required.
