The Community Action Collaborative (CAC) works together in an attempt to reduce the number of emergency room admissions, and jail admissions, reduce overall response costs, reduce costs for health care, increase connections with necessary resources, reduce duplication of services, decrease identified burdens to multiple numbers of agencies and resources created by untreated/under-treated individuals. The CAC incorporates the philosophy of looking at preventative and diversion interventions, looking at how to do something different, and reducing silos of interventions in a collaborative and inclusive environment. This collaboration is a combination of North Range Behavioral Health, Northern Colorado Health Alliance, and Greeley Fire's community paramedic

Our Bachelor Level Mental Health Counselors help clinicians provide initial and ongoing assessment, crisis intervention, individual, family, and group therapy, and case management services. They aid in developing strength-based service plans, monitor treatment progress, document interactions, and maintain electronic clinical records and other reporting data for individuals with primary mental health challenges.

Duties Performed: Adhere to utilizations, quality improvement, and corporate compliance policy and standards. Provide services to clients as assigned by administrative/clinical supervisor. Provide person-centered assessment, goal planning, and treatment for mental health issues. Provide screening, intakes, and assessment of clients utilizing required instruments and formats within appropriate timelines. Develop strength-based written treatment plans/updates and continuing care plans and make appropriate referrals. Complete written discharge summaries and correspondence within established timelines. Assess the diagnosis and mental status of clients, evaluate the current level of functions, and develop and implement treatment/service plan to address clients' needs. Intervene and de-escalate crises with clients. Provide crisis intervention via telephone or face-to-face in a variety of settings that may include crisis, walk-in services, mobile on-site services, residential settings, etc. Assess the risk of harm to self or others and arrange needed resources. Collaborate with other clinicians, law enforcement, external providers, and psychiatrists to provide services to clients. Maintain close and cooperative relationships with community agencies and other public agencies in carrying out treatment/service plans. Collaborate continuously with assigned clients to devise safety planning, provide symptom management, identify support systems, and reduce and mitigate risk factors. Assist individuals in improving their well-being and quality of life by utilizing therapeutic interventions, and training in behavioral skills, educate clients to make informed choices; develop clients' coping skills; help clients to problem solve and negotiate tasks of daily living.
