Under the direct supervision of the Nursing Manager, and in collaboration with the interdisciplinary team, the Registered Nurse uses clinical judgment skills to access and implement the treatment plan for patients. The RN assesses monitors, detects, plans and takes appropriate action to prevent physiological complications associated with specific health problems and/or medical treatment.
Coverage areas include:Northern Kent County, Newaygo, Montcalm, Mecosta, and Southern Portions of Lake and Osceola Counties
Education - Associate's Degree or equivalent in Nursing
Experience - 2 years of relevant experience in OB-postpartum and breastfeeding support, specific to the Going Home RN for Healthier Communities
Registered Nurse (RN) State of Michigan
Drivers License – MI (MI DR LIC) and car insurance; Specific to VNA,Home Care, Hospice, and Carol Health Upon Hire
Basic Life Support (BLS) Specific to VNA and Home Care, 90 Days
Education - Bachelor's Degree in Nursing
Experience -Minimum 2-3 years nursing in Palliative Care, hospice, home care, oncology or ambulatory care
Knowledge of end-of-life care, including advance care planning, symptom management, and bereavement care a plus.
