About the Division of Youth Services: The Division of Youth Services (DYS) provides a continuum of residential and non-residential services that encompass juvenile detention, commitment, and parole. DYS is the agency statutorily mandated to provide for the care and supervision of youth committed by the District Court to the custody of the Colorado Department of Human Services. The Division operates numerous secure facilities that serve youth between the ages of 10 and 21, who are pre-adjudicated, sentenced, or committed. The Division also contracts with numerous private residential and non-residential service providers throughout the State. For pre-adjudicated youth, the Division is also responsible for the management and oversight of Colorado Youth Detention Continuum, a State-funded, locally administered program that provides services to youth at risk of further progressing into the juvenile justice system. In addition to residential programming, the Division administers juvenile parole services throughout the State which includes but is not limited to parole supervision through case management to ensure community safety.

Description of Job

Grand Mesa Youth Services Center:

This work unit exists to provide a safe, secure, and healthy environment for all youth who are committed to the secure treatment facility. Services are provided consistent with evidence-based practices and the agency's Sanctuary model and trauma-responsive environments. Services include transition programming to youth across the campus and collaboration with Behavioral Health Specialists and other members of the Multi-Disciplinary team. This program works to safely and efficiently treat and return youth back to the community by providing a trauma-responsive environment while ensuring safety for youth, staff, and the community.

This position is responsible for the successful integration of committed youth into the community. This position addresses the following needs: individual case management, employment, education, transportation, housing, job search, ID cards, birth certificates, Behavioral Health services in the community, mentor connections, faith-based partnerships, community supervision when needed, and transition classes. Through collaboration with Multi-Disciplinary teams, transition plans are developed and services are identified based on individual needs. The focus is to ensure the successful reintegration of youth back into communities while promoting protective factors that increase support and resources. Collaborates with internal stakeholders as well as workforce development centers, community-based organizations, schools, and employers.

Job Duties:

Transition Services

  • Prepares and maintains a case folder for each assigned youth using a Google folder or a similar electronic method to store and share transition case folders with other members of the MDT/Transition Team.

  • Conducts initial interview and assessment of youth in transition within five working days of referral.

  • Discusses with youth how such issues as substance use disorders and anger management problems might play a role in their community activities and transition support needs.

  • Facilitates research-based life skills assessment and curriculum for transitioning youth, facilitating life skills group weekly for youth in transition.

  • In collaboration with the MDT, develop measurable transition goals based on the ITP objectives for the caseload of youth on Transition Status.

  • Incorporates a review of youth risk/needs/responsivity requirements to build an effective transition plan, incorporating the Roadmap to Success and Transition Checklist.

  • In collaboration with Behavioral Health Specialists and program staff, assists youth with the completion of the youth portfolio.

  • Meets with youth to evaluate their progress in accomplishing goals and maintaining the terms specified in their transition plans.

  • Informs youth of the requirements of temporary release passes, such as safety plans, sponsorship, schedule and attendance documentation, paycheck management, restitution payments, or educational and employment stipulations.

  • Completes all necessary transition paperwork for off-site events.

  • Monitors, evaluates, and documents progress and communicates regularly with youth’s Behavioral Health Specialist and Client Manager.

  • Utilizes outside stakeholders and community resources to assist the family and youth in setting up proper services for the youth’s parole.

  • Assists transitioning youth in enrolling for medical, mental health, or substance use treatment services according to individual needs.

  • Arranges for post-release services such as employment, housing, counseling, education, and social activities in collaboration with Behavioral Health Specialist, Client Manager, and MDT members.

  • Assists youth with the transition back to a local school if applicable through attending safety plan meetings, best interest determination meetings and/or providing data and information as requested, and working with the principal in DYS to ensure the transition is known and planned for.

  • Communicate to identified school personnel transition goals, processes, and timelines through email or other formal processes.

  • Helps youth develop strategies to promote their own wellness and independence.

  • Responsible for community networking and transitional services to include community meetings, school interviews, internships, job placement options, and life skills programming to effectively re-integrate the youth back into the community.

  • Actively participate in treatment team meetings, as well as other appropriate meetings.

  • Assists in scheduling client transportation for assigned clients.

  • Advises youth regarding food stamps, childcare, food, money management, sanitation, or housekeeping.

  • Investigates alleged violations of terms of community transition passes, using interviews, schedule, and attendance records.

  • Ensures the family is updated on youth’s progress via phone contacts and monthly meetings with the client manager and family.

  • Identifies and/or develops sustainable community resources that are available during transition and once DYS is no longer involved.

  • Participates on boards, committees, and planning groups relevant to transition and aftercare.

  • Collaborates with other DYS Transition Coordinators to improve both the quality and consistency of transition services statewide.

Safety and Security

  • Responsible for safety and security procedures and daily operations when working with youth.

  • Schedules and coordinates transportation for assigned youth and/or does the transportation themselves.

  • Implements DYS Safe Correctional Practices.

  • Is aware of the risks associated with taking youth off campus.

  • Supervises youth during off-campus medical, treatment, or court appointments.

  • Follows protocol for taking youth off campus and when in the community.

  • Is trained as an informed supervisor in regard to sex offense-specific youth and is knowledgeable about a youth’s specific risk issues.

  • Evaluate each youth’s safety plan prior to leaving the facility to ensure heightened awareness of youth’s specific risk areas and triggers.

  • Monitors youth’s physical and emotional well-being during regularly scheduled meetings and transition passes and reports unusual behavior or physical ailments to BHS, medical, and youth center staff.

  • Uses appropriate verbal de-escalation and other skills to ensure safety as needed.

  • Trained in First Aid/CPR and approved physical interventions and utilizes as necessary.

Training

  • Attends all assigned yearly training in accordance with DYS policies and training calendar of mandatory training.

  • Remains current in CPR/First Aid and PSI training.

  • Is trained in Case Conceptualization, Motivational Interviewing and Trauma responsive Environments, and the Sanctuary Model.

  • Fully utilizes all of these trainings in daily interactions with youth.

  • Attends Roundtable meetings as scheduled.

  • Provides in-services or other training in the area of transition services as requested.

Minimum Qualifications:

Experience Only:

Six (6) years of relevant experience in an occupation related to the work assigned to this position

OR

Education and Experience:

A combination of related education and/or relevant experience in an occupation related to the work assigned equal to six (6) years.

Preferred Qualifications

  • Work experience with and knowledge of Western Colorado community agencies, judicial system, law enforcement, education system, and service providers.

  • The position requires an individual who is self-directing and motivated, and who can make decisions with limited supervisory direction.

  • Must be flexible and comfortable with change.

  • Knowledge of computers and data entry.

  • Bilingual

Conditions of Employment:

  • Be a minimum of 21 years of age, with no felony convictions or history of domestic violence

  • A pre-employment criminal background check and drug screen will be conducted as part of the selection process as this position is deemed to have direct contact with vulnerable persons. Felony convictions, conviction of crimes of moral turpitude, or convictions of misdemeanors related to job duties may disqualify you from being considered for this position. Should your background check reveal any charges or convictions, it is your responsibility to provide the CDHS Background Unit with an official disposition of any charges.

  • Travel within the State of Colorado to meet with community partners and clients who are within the Western Region Catchment area.

  • Possess and maintain a valid unrestricted US driver's license

  • Must be available by phone in case of urgent matters and emergencies to answer questions, provide input or complete necessary paperwork.

  • Continued employment is contingent upon successful completion of the DYS Pre-service Training Academy, Field Training Program (FTO) and mandatory annual training in all subject areas.
