Job Summary

We are seeking a compassionate and experienced Hospice Nurse Practitioner (NP) in the Grand Haven, MI Area to join our dedicated hospice care team. The NP will provide high-quality palliative care to patients facing life-limiting illnesses, working closely with interdisciplinary team members to ensure comfort, dignity, and optimal quality of life for patients and their families.

Key Responsibilities :

  • Patient Care :

  • Perform comprehensive patient assessments, including physical exams and symptom management.

  • Develop and implement individualized care plans in collaboration with the hospice team.

  • Manage and adjust pain and symptom control treatments to improve patient comfort.

  • Provide emotional support and guidance to patients and their families during end-of-life care.

  • Collaboration :

  • Work closely with physicians, nurses, social workers, and chaplains to provide a holistic approach to hospice care.

  • Participate in care conferences to discuss patient progress and modify care plans as needed.

  • Education :

  • Educate patients and families about disease progression, hospice services, and end-of-life care options.

  • Provide guidance on advance care planning, including advance directives and DNR orders.

  • Documentation :

  • Maintain accurate and timely medical records in compliance with hospice policies, Medicare guidelines, and legal requirements.

  • Complete certification of terminal illness documentation as necessary.

Qualifications :

  • Master’s or Doctoral degree in Nursing from an accredited NP program.

  • Active Nurse Practitioner license in the state of Michigan

  • Certification as a Hospice and Palliative Care Nurse Practitioner preferred

  • Minimum of 5 years nursing experience; clinical nursing care in oncology, hospice, geriatrics, intensive care preferred

  • Strong clinical assessment and critical thinking skills.

  • Excellent communication and interpersonal skills with a focus on compassionate care.

  • Ability to work independently and as part of a collaborative team
