The Impact You Can Make * *Team Impact * To provide and document nutrition interventions including nutrition counseling, nutrition education, and care continuation utilizing the nutrition care process. * * *The Glens Falls Hospital Impact Mission * Our Mission is to improve the health of people in our region by providing access to exceptional, affordable, and patient-centered care every day and in every setting. * * *How You Will Fulfill Your Potential Responsibilities * Comprehensive nutritional assessment of patients prioritized to be at risk during the screening process or as a requested consult. * Develop appropriate interdisciplinary care plans based on current treatment knowledge and clinical practices, discussing care plan with patient and/or significant other and the appropriate staff. * Respond to requests for nutrition education or counseling for patients and/or groups including appropriate materials for continuation of medical nutrition therapy or wellness. Education/Accredited Programs * B.S. Degree in nutrition or dietetics. * Registration through the Academy Of Nutrition & Dietetics. * State certification as Certified Dietitian/Nutritionist. * Master’s Degree preferred Licenses/Certifications/Registrations * Achieve and maintain dietetic registration. * Certification by the New York State Education Department as Dietitian/Nutritionist. * Certification as Diabetes Educator or Nutrition Support Clinician preferred.
