JFCS Administration Adult Case Manager (East Valley) in Gilbert, Arizona

Critical Tasks

The Adult Case Manager (ACM) provides on-going case management services to members who have been designated as Seriously Mentally Ill (SMI). The ACM will perform out-of-office case management services and administrative tasks in accordance with agency procedures and the Arizona Principles for working with SMI clinical teams. The ACM performs duties out in the community and is expected to provide transportation, when necessary, to members and coordinate transportation services, as appropriate.

  • Coordinate services for behavioral health and coordination of care with physical health care providers.

  • Working knowledge of common and uncommon mental & behavioral healthcare disorders including, Schizophrenia, Bipolar Disorder, Major Depressive Disorder, PTSD, Personality Disorders, Substance Abuse Disorders, etc.

  • Maintain communication, coordination, and connections with all Clinical Team participants.

  • Working knowledge of all Mercy Maricopa Protocols for serving adults with SMI in a clinical team setting.

  • Ensure implementation of “transition into adulthood” referrals.

  • Understand and navigate the rights of the patient regarding access to treatment, appeals and complaint processes instituted by the Mercy Maricopa RBHA.

  • Manage a caseload of 1:30, not to exceed 1:40 for Supportive Level of Care.

Key Performance Indicators

  • Ability to write clinically sound comprehensive assessments and Integrated Individual Service Plans (IISP).

  • Knowledge of techniques and approaches to identify the adult’s underlying, unmet needs and can differentiate needs from services.

  • Ability to work comfortably in a clinical team setting within the member’s home and community.

  • Ability to recognize member strengths and to state strengths in functional terms.

  • Familiarity with P.M. Section 5.1 Notice Requirements and Appeal Process for Title XIX and Title XXI Eligible persons; and P.M. Section 5.2 Member Complaints.

  • Demonstrated experience using the following skills:

  • Organization and follow-through

  • Monitoring the delivery of services identified in the member IISP to ensure integrity and effectiveness.

  • Documenting into the medical record in accordance with contractual medical record standards

  • Recognizing and seeking clinical consultation when clinically appropriate

  • Developing and supporting the implementation of crisis and safety plans as clinically indicated

  • Taking into account the cultural preferences of the member

Metrics

  • Successful facilitation Adult Clinical Teams/Staffing’s, i.e. reframing, active listening, solution-focused, modeling pro social behavior.

  • Maintains accurate Electronic Medical Records (EMR) in NextGen.

  • Completes Electronic Medical Records (EMR) in NextGen in a timely manner to meet access to care standards.

  • Meets required productivity standards as defined by the Agency.

  • Remain current and compliant with all assigned Relias trainings.

Qualifications

  • Education / Experience Requirements:

  • Bachelor’s Degree in Behavioral Health field or,

  • Bachelor’s Degree in non-Behavioral Health field + 1 year of Behavioral Health experience.

  • Associates Degree + 2 years of Behavioral Health experience

  • Diploma / GED + 4 Years of Behavioral Health experience

  • Ability to pass DPS Level 1 Fingerprint Clearance without any restrictions

  • Reliable Transportation

  • Active / Unrestricted AZ Driver’s License

  • Bilingual English / Spanish Preferred
