Responsibilities

JOB SUMMARY

This job is responsible for overseeing patient placement operations to assure that all aspects of patient access, placement, and flow are carried out in a timely manner in accordance with CHI Franciscan Health (CHI-FH) policies and procedures and applicable nursing care standards and regulatory requirements. Work is performed through continual monitoring of the Mission Control Wall of Analytics (WOA) made up of tiles that process/display real-time data from multiple source systems, including patient location, acuity, risk factors, turnaround times and real-time scheduling data. Work includes mentoring/guiding staff on the assigned shift to plan, prioritize, organize, direct and control all admissions and transfers between nursing units, external hospitals, Behavioral Health, Emergency Dept, PACU, procedural areas as well as direct admissions from provider offices/clinics.

Work includes: 1) serving as liaison between on-site bed management staff, Mission Control team members and the Transfer Center RN on all issues pertaining to patient placement; 2) maintaining ongoing communication with referring providers and transferring hospitals regarding bed availability/status and/or access delays; 3) predicting future patient placement pressure points and participating in the execution of over-capacity protocols; and 4) initiating interventions, within scope of practice, to accelerate patient flow or other mitigate barriers to optimal throughput.

Job duties require the services of a Registered Nurse who, based on training and work experience, uses professional knowledge and judgment in understanding and interpreting the clinical acuity/diagnoses of patients and integrating that data with real-time and predictive regional bed availability information to ensure that patients are timely matched with the appropriate facility, level of care and unit.

ESSENTIAL JOB FUNCTIONS:

Manages, in collaboration with Mission Control Patient Placement Reps, all patient placement decisions in based on patient diagnosis, acuity and location, applicable risk factors and unit criteria/bed availability.

Works with staffing and/or nursing teams to adjust staffing and implement bed management plans in conjunction with the Nursing Supervisor and/or Nurse Manager.

Mentors and provides guidance to Patient Placement Reps and assures adequate staffing levels to meet Mission Control needs.

Works collaboratively with Mission Control Center team and other stakeholders to address, moderate and/or resolve real-time and predictive future patient placement flow and related issues.

Qualifications

Education/Work Experience Requirements

Bachelor’s degree in nursing or other healthcare-related discipline, graduation from an accredited school of nursing, and five years of related clinical quality/performance improvement work experience in a health care setting that demonstrates the attainment of the requisite job knowledge, skills and abilities.

Equivalent related work experience may be substituted for the Bachelor’s degree requirement.

Master’s degree in a discipline related to the requirements of the position is preferred

Licensure/Certification

  • Current licensure as a Registered Nurse through the Washington State Board of Nursing.

Pay Range

$42.69 - $70.18 /hour
