CaroMont Health Specialty Navigator RN in Gastonia, North Carolina
Job Summary: Provides patient centered navigation services and care coordination across the continuum of care. Participates in assessing, planning, implementing, and evaluating health services of a disease specific population during hospitalization, post discharge, during care through specialty clinic and site of service. Performs tasks requiring independent knowledge and judgment. Implements performance improvement initiatives. Provides community outreach activities. Serves as a liaison and resource among patients, families, healthcare providers, staff and community to provide a seamless transition throughout phases of care. Reports data and implements quality improvement initiatives focusing on readmissions, complications, and patient self-management skills through the CaroMont Service Lines.
Qualifications: Bachelor’s Degree from a recognized college or university required. Master’s Degree preferred. Current RN license to practice in NC (NC license or multi-state compact license). At least three years of acute patient care in nursing.. Experience or knowledge in case management, specialty disease, or navigation preferred. Navigation or Specialty certification a plus. Requires ability to work in a self-directed environment to work with the multidisciplinary team to communicate with all members on behalf of the patient. Excellent communication, organizational skills, critical thinking, problem solving, interpersonal, customer service, teaching and coaching skills. Proficiency in Microsoft Programs. Requires ability to use data to prepare reports (including charts and graphs). Requires critical thinking skills to analyze and review reports to improve patient outcomes. BLS required. Certification/Advanced Disease Education as required by Service Line Leadership (Once testing requirements are met, testing/certification must be completed within 12 months).
