Minimum Qualifications:
Bachelor's degree in Psychology, Social Work or Mental Health / Behavioral Science. Criminal Justice degree considered with 15 hours of coursework in psychology, social work and/or counseling with 2 years of MH case management experience.
Job Summary:
This position is responsible for providing case management, social and habilitative services, and counseling to patients. Works under the supervision of a Psychologist, Mental Health Manager or Program Director.
Job Duties:
Collaborate with program clinicians to coordinate ongoing care for TCHATT patients
Act as liaison between patients and clinicians
Disposition TCHATT cases upon discharge from services
Develop and coordinate care plans for patients
Assist with development and maintenance of the mental health resource database
Demonstrate competency and skill in communicating with patients, families, and school personnel
Participate in team meetings and provide status updates to team members and leadership
Participate in conferences, workshops, and trainings related to your role
Assist with maintaining project data and required reports
Other duties as assigned
Preferred Qualifications:
1 year experience working with families.
Must have proficient computer skills.
Salary Range:
Actual salary commensurate with experience.
