PACE (Program of All-Inclusive Care for the Elderly) was developed to answer the many problems around caring for frail seniors. The PACE model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in their community whenever possible.

Seniors, their family members, and caregivers face many issues, including transportation to appointments, management of medications, coordination of medical care from different specialists, lack of social interaction, and ability to stay alone at home. On Lok PACE participants receive in home care services and transportation to a On Lok PACE center for primary medical care, social and recreational activities, and other senior care services.

POSITION SUMMARY: Under the direction and supervision of the Supervisor/Manager, the Caregiver 2 provides personal care services to On Lok participants receiving home care and/or day center services. At On Lok, Caregivers provide the same services as a "Personal Care Attendant" position referred to in the PACE regulation (42 CFR Part 460).

Caregivers will need to have experience working within the senior population and an applied understanding of techniques and the activities of daily living within this demographic. CNA or HHA certification highly desirable.

DUTIES / RESPONSIBILITIES:

  1. May perform duties of a Caregiver 1.
  2. Provides onboarding and ongoing training for other Caregiver-1s and 2s using manuals and formal training materials as a guide.
  3. Active participation in departmental meetings and family meetings as needed.
  4. Other related duties as assigned.

QUALIFICATIONS (knowledge, skills, abilities):

  1. Minimum one-year experience working with frail or elderly required.
  2. Minimum one-year experience working in home or clinic setting preferred.
  3. HHA/CNA required; On Lok experience in lieu of certification may be considered.
  4. Understanding of PACE model gained through personal experience or research.
  5. Must be flexible and have a positive attitude working across multiple settings.
  6. Ability to work independently and as part of interdisciplinary team.
  7. Must have current Basic First Aid and CPR certification.
  8. Knows, uses and explains use of safety equipment for lifting; corrects others in a positive way "here let me help you and show you how this works".
  9. Physical requirements: Ability to stoop, reach, stand, walk, push, pull, lift, grasp, talk, hear, and repeat motions of the wrist and hands. Must have visual acuity. Must be able to lift/move weight that is within individual physical ability and use appropriate lifting devices or request assistance when needed.
  10. Basic English reading and writing required; bilingual Cantonese required for specific assignments and daily participant interaction.
  11. Basic computer skills; able to use or learn to use multiple applications such as email, electronic health record.

Your final compensation offer will be determined based on factors such as skills, experience, education, and licenses/certifications. The pay range is expressed as an hourly rate regardless of FLSA status. Exempt positions are paid salaried and are not subject to overtime.
