Heartly House, Inc. Lead Service Coordinator in FREDERICK, Maryland

**LEAD SERVICE COORDINATOR

This is an exempt, full-time position that reports directly to the Residential Services Director.

AREAS OF RESPONSIBILITIES

Assists the Residential Services Director with implementing all trauma-informed shelter, housing, and service coordination programs and services

Provides on-call teaming about emergency client needs and requests for services to residential staff and substitutes on a rotating schedule with the Residential Services Director

Assists the Residential Services Director with preparation of grant applications and statistical and narrative program reports for residential programs

Assists the Residential Services Director with inventory and management of food, supplies, and donations and makes recommendations for needed items

Provides service coordination to clients in Heartly House shelter and housing programs and clients receiving services through other agency departments, as directed

Provides shelter coverage and direct client services during isolated shifts including evenings and/or weekends to ensure availability to clients

Conducts client screening, intake, and needs assessment for Heartly House residential programs

Develops, implements, updates, and evaluates individual client service plans on an ongoing basis

Assists clients with information/referrals to appropriate service resources including but not limited to housing, health, education, training, childcare and/or employment

Advocates on behalf of clients to ensure access to service resources outside of Heartly House

Enforces program policies and procedures

Provides crisis intervention to residential clients

Mediates conflicts between residential clients

Conducts weekly house meetings and room inspections with shelter clients

Conducts housing unit inspections prior to move-in and during reevaluations with housing clients

Keeps accurate case and financial records

Maintains client confidentiality at all times

Maintains shelter security at all times

Meets with Residential Services Director regularly

Attends Heartly House staff and departmental meetings as employment schedule permits
