UPMC Home Care Aide (Venango) in Franklin, Pennsylvania

Responsibilities:

  • Communicates appropriately with the interdisciplinary home care team promptly, reporting any changes in patients' condition to appropriate professional staff.

  • Attends compliance training and adheres to the organization's standards of conduct, policies, and procedures.

  • Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction.

  • Provides safe and direct personal care to patients in the home setting under the direct supervision of appropriate professionals.

  • Performs duties and job responsibilities in a fashion that coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians, and other departments within the organization.

  • Identifies, develops, and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide.

  • Proven ability to facilitate change.

  • Maintains and completes documentation accurately and promptly while in the patient's home and within the agency and regulatory standards.

  • Participates in all required agency meetings.

  • Must maintain annual competency evaluation scores as determined by regulatory requirements.

  • High School Diploma or GED - Required

  • One (1) year experience - Preferred

  • Certified Nurse Aide - Preferred

  • Minimum of 75 hours of Nurse Aide training - Required. If no previous nurse aide training, the applicant may attend the UPMC Home Healthcare Nurse Aide Training Course as available upon hire.

Licensure, Certifications, and Clearances:

Preferred: Nurse Aide Registry in good standing either in the state where the facility is located or in a state covered by a licensure compact agreement with the state where the facility is located. Required: CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire

  • Automotive Insurance

  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)

  • Driver's License

  • Act 33 with renewal

  • Act 34

  • OAPSA
