Cardiovascular Program Coordinator

Work hours: Full-time, day shift, 40 hours weekly

At Texas Health Southwest and Texas Health Clearfork we’re one hospital in two locations. We’re a 261-bed, full-service hospital serving southwest Fort Worth and surrounding areas since 1987.
We specialize in surgical services, orthopedics, rehab services, imaging services and adult critical care, and we have a 24-hour emergency department. Plus, we are a certified Primary Heart Attack center, certified Primary Stroke center, Level III Trauma, Level III Maternal, Level III Neonatal center, certified Total Hip and Total Knee Replacement facility, and a nationally accredited Breast Center.
Texas Health Southwest has received Magnet designations, plus a CMS 5-Star rating, the US News & World report top hospital award, and multiple awards for STEMI/NSTEMI.
Here’s what you need
Education
Bachelor's degree in nursing required and
Master's degree in nursing preferred
Experience
Three (3) years Med/Surg, Emergency or Critical Care experience required and
Three (3) years emergency or critical care experience preferred
Licenses and Certifications
RN - Registered Nurse upon hire required and
SCRN - Stroke Certified Registered Nurse or other nursing professional certification upon hire preferred
Skills
Effective leadership and strong interpersonal skills; verbal and written skills in English. Advanced knowledge of current professional and health care issues and nursing practice.
What you will do
Stroke/Chest Pain:
Analyzes data, distributes reports to appropriate team members and administrators
Manage data collection
Establishes and maintains a multi-disciplinary CQI program to include morbidity / mortality review
Assists in establishing standards of care, policies, and objectives of stroke/chest pain services
Quality:
Establishes and Maintains Appropriate Special Programs Designation
Fulfills the standards for chest pain and stroke care as required by the appropriate surveying agencies, (i.e., Texas Department of Health and the Joint Commission)
Collaborates with chart abstraction team to ensure accuracy and timely reporting
Staff Development/Education:
Coordinates and facilitates provision of educational opportunities for staff, physicians, patients, external providers of care, and the community
Identifies relevant areas, initiates projects and participates on performance improvement teams
Professionalism:
Represents department in a positive manner to hospital/system/community
Maintains current and active participation in professional organizations
Teamwork and communication:
Works effectively and productively with all members of the patient care team as well as family members/significant others and pre-hospitalization stakeholders
Collaborates with medical and administrative leaders to ensure positive relationships with local EMS agencies
Oversees and facilitates care and follow-up of patients
