Community Health Systems Care Manager RN - Weekend Premium Approved in Fort Wayne, Indiana

Under the direction of the Division Director, Case Management, serves as a coordinator of an interdisciplinary approach to guide patients through the continuum of care with special focus on the clinical components of care/medical management, utilization/resource management and insurance issues. Collaborates with the physicians, patients, their family members and others to facilitate the achievement of desired outcomes, address ongoing needs, maximize patient, family and provider satisfaction, optimize resource utilization, facilitate timely discharge, assure appropriate length of stay, reduce unnecessary delays/other adverse variances to delivery of services, and assess physician quality for peer review activities.

Minimal Acceptable Job Qualifications

Experience : Minimum of one to two years of clinical experience in nursing practice

Education : Completion of an accredited nursing program.

Licensure/Registration/Certification: Currently licensed in the state of Indiana as an RN .

Job Knowledge & Skills : Requires strong knowledge of multidisciplinary care planning, nursing scope of practice, and problem-solving.

Demonstrates the knowledge and understanding of intensity of service, severity of illness, opportunities for intervention, planned course of treatment/procedures, care needs, and outcome goals.

Demonstrates an understanding of the role of payors and providers, the need for cost containment and the effect of appropriate resource utilization.

Demonstrates strong analytical and organizational skills to coordinate patient services in diagnosis based groups to assure appropriate utilization of health care resources, increase team activity to identify and implement utilization refinement, improve patient and physician satisfaction, and increase physician awareness of the impact of inappropriate utilization of resources and the need for collaboration and compliance with hospital expectations.

Demonstrates an extremely high level of interpersonal and communication skills, both verbal and written, in order to interact effectively with all internal and external customers.

Demonstrates working knowledge of the English language verbal and written.
