What is the ISS program?

We support individuals who live independently in the community to maintain and expand their independence and pursue their dreams. We design and implement supports for each individual according to their specific needs and desires. Our supports include medical coordination, financial coaching, community integration, emotional/behavioral supports, and many others.

What does a Person-Centered Plan Manager do?

• The Person-Centered Plan Manager, also known as a case manager, works directly with the individual in the community to help them achieve their goals. Typically, approximately 70% of time is spent in the community.

• Connects the individual with the services they need to maintain and expand their independence. This may include community services, government-funded benefits, or other publicly available resources.

• Advocates for the individual ensuring that they receive the proper support for their situation.

• Completes administrative tasks on behalf of the individual, including consultations, documentation into an electronic record system, benefits eligible forms, and other related tasks.

Qualifications:

• Bachelor's Degree or equivalent experience, preferably in a related field.

• Valid driver's license with an acceptable driving record and ability to pass background checks.

• Basic computer skills, including Microsoft Office, email communication, internet, and documentation entry.

• Ability to work independently and collaboratively as part of a team.

• Compassion, resourceful, and a strong desire to learn and grow.
