Summary: The PCPM collaborates with program participants to develop individualized ISP goals including all accompanying documentation and increase employability and community skills. Supports a caseload of program participants by maintaining ISPs, serving as a liaison for participants and supervising Support Specialists. The PCPM works collaboratively with the Program Director for oversight of the staff and for overall support.
Schedule: Monday-Friday 8am-4pm (40 hr/week)
Responsibilities:
• Ongoing written and verbal supervision of Support Specialists
• Manage ISP related documentation to remain compliant with DDS standards as well as HCSIS and iCentrix
• Timely and accurate coding for services into electronic medical records system for billing purposes.
• Maintain a caseload of participants across all Community Based Day and Employment Support programs
• Works with individuals with developmental and intellectual disabilities (DD / ID), and Autism Spectrum Disorder (ASD) population.
• Serve in staffing ratio when the need arises
• Responsible for implementing key objectives, ensuring high standards of direct care and ongoing monitoring and management of operations in assigned community and employment programs
. • Collaborates with others in the planning, management and fulfillment of agency short/ long term vocational services CBDES goals.
• Completes all administrative functions for assigned programs, compliancy, supported individuals, agency and direct reports on time
• Completes and submits required documentation, which includes daily production sheets, monthly Progress Notes, ISP documentation, incident reports, accident reports, seizure reports, and employee report of injury using computer technologies and applications for on-line submission and tracking
• Enhances and promotes The Arc of Opportunity's strategic initiatives, overall brand and organizational mission and values through positive promotion and representation.
• Maintains required certification and recertification as required and in accordance with agency, state and federal guidelines
• Driving of a motor vehicle and/or van, agency and/or personal
• Administer medication in accordance with MAP regulations
• Other duties as assigned.
Qualifications:
• A valid driver's license and acceptable driving record and CORI are required at all times during employment with The Arc of Opportunity
• Bachelor's Degree in relative field or equivalent experience.
• A minimum of two years supervisory experience required.
• A valid driver's license and acceptable driving record and national background check is required
• Experience accessing community resources is preferred.
• Ability to perform physical restraint if necessary.
• Ability to lift up to 50 lbs., kneel, bend, and carry items, able to sustain periods of walking, standing and sitting.
• Functional and situational reasoning to follow policy, problem solve, exercise sound judgement, prioritize multiple tasks and crisis manage
• Functional and situational reasoning ability to gather and apply factual information, exercise sound judgment, problem solve and apply principles of influence systems such as motivation, team building, accountability, coaching/counseling of staff and leadership within the scope of the position.
• Cross-functional communication skills, written and oral, which convey clear direction, desired results and vision to direct reports, individuals/families served, government agencies, peers and senior management.
• Computer literacy.
