    What Programs Does the Division Director of Nurse Case Management Oversee?

    The Division Director of Nurse Case Management supervises a team of nurses within our Residential Programs, Day Habilitation, and Adult Foster Care. Below are descriptions of the programs:

  • Residential Program: With 13 residential programs, our community home offer models serving 4-5 individuals in a continuously supported and staffed environment, ensuring individuals are encouraged and offered opportunities to become valued members of the community.

  • Day Habilitation: Mass Health-funded medical modeled program that focuses on a variety of therapy combined with clinical supports that creates an environment that inspires personal growth and development for each of our participating individuals.

  • Adult Foster Care (AFC) Program: a medicaid-funded initiative that enables individuals with disabilities to live with caring providers in a safe and nurturing environment. The program provides comprehensive case management and nursing oversight.

    What Does the Division Director of Nurse Case Management Do?

  • Lead nursing functions across the organization

  • Provide direction and consultative support to team members and program leadership.

  • Build and execute outcome-based training programs to ensure quality of medical and nursing care

  • Develops nursing and healthcare policies and procedures that comply with all regulatory, licensing, and accreditation standards.

  • Lead Healthcare Committee, participate in meetings and initiatives to enhance service delivery; serves as member of PBS Leadership Team and other committees as assigned.

    Qualifications:

  • Registered Nurse, registered with the MA Board of Registration in Nursing required

  • BSN required, MSN preferred

  • Minimum 5 years supervisory experience in a community health setting with at two years of supervisory experience

  • Prior experience working with persons with intellectual and developmental disabilities

  • and/or acquired brain injury
