POSITION: RN/LPN Unit Manager

DEPARTMENT: Nursing

RESPONSIBLE TO: Director of Nursing/Assist Director of Nursing

JOB SUMMARY:

  • Plans, implements, and coordinates all nursing for unit, including nursing services for SNF resident on unit.
  • Supervises and evaluates the performance of staff nurses, CNAs and Unit Secretaries who work on unit.
  • Assists In-service Director in coordination of orientation of new trainees and Inservice on nursing department employees on unit.
  • Assists DON in assuring quality nursing care and agency compliance with Medicaid, Medicare, and State Health requirements.
  • Responsible for promoting and maintaining good interpersonal/interdepartmental relationships, including carrying out administrative procedures in conformity with BRNC policy.
  • Responsible for enhancing professional development of staff in areas of creativity, skills behavior, and personal appearance.
  • Responsible for keeping DON and Supervisors fully informed regarding needs, unusual occurrences, problems, and accomplishments to establish/maintain lines of communication and to enhance continuity of quality resident care.
  • Performs nursing duties of staff nurse, including charge nurse duties, as required.

SPECIFIC DUTIES:

  • Plans effective administration of all nursing services on unit.
  • Coordinates through DON, Supervisors and Staff Nurses all unit management and resident care tasks.
  • Responsible for knowing Fire, Safety and Disaster Manuals, including direction of subordinates in prevention drills, emergencies, and other related situations.
  • Responsible for care/utilization of supplies/equipment on unit.
  • Assures physicians orders are properly transcribed and implemented with appropriate supporting nursing documentation.
  • Assures appropriate MD notification per policy and within professional standards, including physicians orders obtained for nursing procedures requiring such an order.
  • When a resident is transferred or discharged, assures complete resident transfer instructions sent to the other facility and written discharge instructions given to the resident and/or family.
  • Assures required monthly summaries are completed with require supporting documentation in compliance with Medicaid, Medicare, and State Health documentation requirements.
  • Validates those medications and treatments that are ordered have appropriate diagnoses and reasons for giving.
  • Assures narcotics and controlled drugs are in correct count for unit.
  • Validates those medications are given as ordered or responsibly withheld, including appropriate documentation and notifications.
  • Assures resident behaviors are documented with appropriate and least restrictive interventions attempted before chemical/physical methods of modification.
  • Maintains current resident problem list of events, occurrences, symptoms, orders, etc. that require follow-up; and validates appropriate documentation and interventions.
  • Validates that CNA Worksheet instructions are updated regarding new admissions and change of condition/orders.
  • Validates that CNA documentation on ADL sheets for Medicare/Medicaid Assessments.
  • Validates that CNA Caregivers Meetings are conducted per policy.
  • Validates that CNA Responsible Party notifications are done and documented per policy.
  • Assess and evaluates nursing service needs on unit.
  • Responsible for in-depth assessment of new admissions and re-admission, including completion of at-risk Assessments (Falls, Wt. Loss, Pressure, etc.). Assures all required documentation (MD orders, History and Physical, Transfer Sheet, Chest X-ray, etc.) arrive with admissions. Responsible for obtaining required documentation from resident/family regarding Preferred Intensity of Care Advanced Directives, No CPR, and use of restraints.
  • Assists MDS Coordinator with assessment of residents proble s and care plan initiation and implementation, including periodic resident reassessment, care plan evaluation and revision within regulatory-set time frame.
  • Responsible for on-going knowledge of and implementation of resident care-plans goals and interventions.
  • Responsible for coordination, evaluation, and documentation of ongoing Restraint Reduction Program and for tracking of side rail usage for QAA Committee.
  • Assures Wound and Pressure Sore development is investigated per policy, assessed weekly (including measurements), and orders treatments and interventions are evaluated and amended, as necessary. Provides written report to DON for QAA.
  • Assures documentation and follow-up of all incidents on unit per policy in timely manner. Investigates all falls, skin tears and bruising, and provide written report to DON for QAA. Assures appropriate interventions for injury prevention in place and in use.
  • Responsible for orientation, supervision, and evaluation of nursing personnel on unit.
  • Evaluates all subordinates on unit on a day-to-day basis.
  • Assists, as delegated by In-service Director or DON, orientation of staff nurses, CNAs and Unit Secretaries to the unit.
  • Assists staff nurses in assignments of CNAs on unit.
  • Assists the DON/ADON with performance evaluations and counseling of and Unit Secretaries.
  • Assists staff in scheduling CNAs for in-service education with In-service Director.
  • Promotes favorable attitude regarding orientation and In-service education.
  • Assists the DON and MDS Coordinator in review of resident charts, including quality indicators to ensure medical and nursing compliance with Medicaid, Medicare and State health requirements.
  • Participates in QAA Committee and Subcommittee Meeting as directed.
  • Participates in Nursing Environmental Rounds and audits for QAA.
  • Promotes harmonious relationships among Administration, co-workers and other departments and services.
  • Responsible for following and fostering use of established lines of communication within the nursing department and between other departments.
  • Ensures personnel conformity to facility policy.
  • Serves as a channel to administration in conveying staffs suggestions for effective change.
  • Assumes responsibility for own professional growth and development through participation in in-service programs, reading professional journals, and attendance at staff meetings.
  • Applies principles of good management to nursing personnel, serving as a role model for implementation and application of these principles in daily work.
  • Provides opportunities for staff to solve problems, to develop management and nursing skills under appropriate supervision.
  • Recognizes individual/group strengths and utilizes strengths to improve weaknesses.
  • Any other duties as requested by DON, ADON or Administration.

JOB QUALIFICATIONS

Graduate of an accredited school of nursing. Current registration with the Virginia State Board of Nursing.

At least one year of experience in supervisory capacity in a hospital or extended care facility, including experience in geriatric nursing.

Good mental and physical health, sound judgment and high moral standards, and a sincere desire to work with those with limited capacity for self-care.

Ability to manage and supervise a nursing unit. must show interest and ability when teaching others and assisting in their skills.

Capacity to grasp new practices and standards and apply their use where appropriate.

Ability to make independent decisions.
