Thompson Health Physical Therapist - Farmington Rehab , Per Diem in Farmington, New York

Schedule: Per Diem, Days

Job Objective: Responsible for evaluation and treatment of individuals with a variety of musculoskeletal and neurological conditions

Key Responsibilities:·

  • Evaluate and treat in outpatient setting.

  • Documentation in accordance with state, federal, JCAHO and insurance guidelines as a per diem

  • Provide skilled outpatient Physical Therapy including evaluation, developing and implementing treatment programs.

Requirements:

  • Valid NYS licensure to practice Physical Therapy

  • Degree in Physical Therapy from CAPTE approved program, DPT preferred

  • Prior experience in outpatient care setting

  • Experience with EMR

  • Team player and self-motivated

Pay Range: $46.00 - $46.00

Starting Pay: Based on experience
