Ohio Valley Goodwill Work Adjustment Program Coordinator in Fairfield, Ohio

For over a century, Ohio Valley Goodwill has been providing support, breaking down barriers, and creating opportunities for individuals with disabilities across Greater Cincinnati.  We offer comprehensive services and training programs that are specifically designed to help individuals with disabilities and other barriers to employment reach their goal of community employment. We help set individuals up for success in every phase of the employment process, from the beginning stages of the choosing the right vocational direction to finding the right employment opportunity and supports while in that job. The Work Adjustment Program Coordinator provides vocational case management services to Work Adjustment participants. Functions as Individual Plan (IP) Coordinator, facilitating team planning and providing linkage to appropriate services. Responsible for case file documentation and management, and developing analyzing of individualized behavior change programming and work skill training in a manner which meets CARF, DODD, and Medicaid standards. MINIMUM REQUIREMENTS: EDUCATION: Bachelor's degree in Vocational Rehabilitation or related field. EXPERIENCE: 1)  At least one year's experience working with individuals with DD and/or relevant case management experience. 2)  Must be able to pass criminal background check and various registry checks.

KNOWLEDGE: Knowledge of positive behavior support principles and training methods for adult learners; knowledge of case management principles, services coordination and team facilitation; and knowledge of Goodwill's emergency procedures. Must possess superior judgment skills in order to ensure the health, safety and well-being of adults with developmental disabilities in a variety of settings. Must pass DMV check, pass road test, DOT physical and pre-employment drug and alcohol test and random testing throughout employment. Must possess sufficient auto insurance to transport individuals enrolled in program. ESSENTIAL DUTIES:

  1. Direct and monitor the implementation of the individual program planning process, involving the person served and others who will be supporting them.
  2. Facilitate all consumer team meetings.
  3. Develop positive and valid training methodologies based on a thorough assessment of individual's strengths, needs, and preferences.
  4. Analyze behavioral/observational data as needed to determine effectiveness of programming.
  5. Document daily, weekly and monthly consumer progress as required per Medicaid, GARF, DOL, and ODDO and verify attendance on the Program Services Record.
  6. Maintain consumer case files in accordance with GARF, ODDO, and Medicaid standards.
  7. Maintain ongoing communications with production supervisors, technicians, instructors, personal care attendants, and others assisting in the implementation of programming.
  8. Conduct service plan reviews with each consumer at least bi-annually, documenting their input regarding programming being implemented.
  9. Assess each consumer's interest in community employment at least on a biannual basis and facilitate community employment options.
  10. Provide staff training on behavior support methodologies, disability awareness, etc.
  11. Make referrals for facility and community-based services as needed.
  12. Responsible for monitoring daily production and bi-weekly client payroll.
  13. Solicit and document consumer feedback regarding program satisfaction.
  14. Update consumer medical and emergency contact sheets.
  15. Transport individuals in personal and/or company vehicles.
  16. Effectively guide and monitor the implementation of programs via direct observation and feedback.
  17. Other duties as assigned.

WORKING CONDITIONS:

Must be able to move around the facility to include all work areas.
