Responsibilities:
- Patient Assessment and Care Planning
- Conduct comprehensive assessments of patients' physical, emotional, and social health.
- Collaborate with the healthcare team to develop individualized care plans based on patient needs.
- Continuously monitor and evaluate patient responses to interventions.
- Maintain accurate and detailed documentation of patient information, interventions, and outcomes.
- Communicate effectively with patients, families, and healthcare team members.
- Collaborate with physicians, nurse practitioners, and other professionals to ensure comprehensive care.
- Advocate for patients' needs, preferences, and rights.
- Provide emotional support to patients and their families throughout the healthcare journey.
- Educate patients on medications, treatment plans, and preventive measures.
- Promote health and wellness through educational initiatives.
* Minimum Qualifications:*
- RN Master's or Bachelor's Degree in Public Health, or Bachelor's in Nursing is preferred
