SUMMARY OF POSITION:
The Clinical Program Coordinator administers and coordinates all multidisciplinary activities associated with the Stroke and Sepsis programs. The individual works in collaboration with the Medical Directors of the individual programs, coordinators at other sites, and nurse leaders. He/she demonstrates knowledge and skills to provide care for the adult and geriatric patient population participating in the Programs.
Chest Pain, and Stroke Accreditation Program and Sepsis Program: This individual is focused on the management of data related to the programs as well as providing necessary support to ensure that program related activities are carried out. Such activities may include: team coordination, chart review, real time evaluation of response and documentation, bedside education for both patients and staff, support groups, educational and promotional events. This individual possesses the ability to display and interpret data. The individual serves as a subject matter expert in the coordination of required accreditation activities.
MINIMUM QUALIFICATION(S):
Minimum of three years of related clinical experience.
State RN License required
PREFERRED QUALIFICATION(S):
Program management or research background preferred.
Experience in Performance Improvement preferred.
Certification in related area preferred
