AIM Independent Living Center OPWDD Family Support Services Intake Coordinator in Elmira, New York

Position Snapshot: The OPWDD Family Support Services Intake Coordinator advocates and assists individuals with developmental and other disabilities with the application process for OPWDD eligibility determination and oversees AIM’s Family Support Services program.

Essential Functions:

  • Welcome and assist all individuals who are seeking OPWDD eligibility services.

  • Explain the OPWDD eligibility process which includes the services and supports once eligibility is determined including the 3-step appeal process for Medicaid and Medicaid Waiver Services, Family Support Services.

  • Provide intake (following the AIM ILC CSR intake process) and collect all documents needed for OPWDD eligibility.

  • Serve as a support for individuals and families new to OPWDD programs and services; help identify choices for service providers.

  • Provide ILS training for OPWDD Family Support Services program including summer camp and monthly classes to Chemung, Steuben, and Schuyler counties for youth ages 3-21. Oversee the Youth Program Specialist in coordination, and scheduling of all children, tween, and youth activities from recreational to ILS training sessions. Including Sib Shop activities for siblings for consumers who have OPWDD eligibility. Providing informational sessions four times per year to parents/caregivers on topics such as community resources, special education, post-secondary, and future planning informational sessions, and supports for people with developmental disabilities.

Education & Experience:

  • Associate degree in Human Services or Social Work with 2 years of applicable experience in human services, case management, customer service, health care, or

  • Any combination of education and experience that provide the knowledge, skills, and ability necessary to perform the essential functions of this position to the satisfaction of AIM’s executive director
