Job Description
MAIN FUNCTION:
A professional nurse who, by virtue of education and clinical experiences, possesses the knowledge and clinical expertise to provide care, education and support to clients. The nurse navigator will provide personalized support and guidance to patients navigating through breast imaging and procedures through diagnosis. This position reports to the mammography supervisor at the Health Center for Women.
DUTIES AND RESPONSIBILITIES:

Quantity of work:
· Performs clinical breast exams on self-referred clients, Breast and Cervical Health Partnership clients, and others as requested
· Collaborates closely with radiologists and technologists to coordinate all components of clinical biopsy procedures, including scheduling, liaison activities with referring physician and radiologist, and patient education and follow-up.
· Assists the radiologist during biopsy and post procedure care.
· Maintains appropriate patient records for clinical aspects of job.
· Keeps all statistics and coordinates follow-up data for all breast cancer cases as required by FDA.
· Provides follow-up for self-referred clients for mammography services and maintains these files for NYSDOH and FDA audits.
· Organized and displays strong computer skills.

Care Coordination:
· Collaborate closely with healthcare providers, including surgeons, radiologists, and other specialists, to coordinate seamless and integrated care for patients with a warm handoff to the Breast Cancer Navigator after a breast cancer diagnosis.
· Facilitate mammography appointments, diagnostic tests, and treatment procedures, ensuring timely access to necessary services.
· Provides emotional support for the client and her family on an individual basis, as well as facilitating support groups as needed.
· Serves as a patient advocate/case manager, helps the individual and her family access services at AOMC and in the community, schedules physician care, and provides coordination of care with the Falck Cancer Center.

Advocacy and Empowerment:
· Serve as a strong advocate for patients, ensuring their voices are heard and their needs are addressed within the healthcare system.
· Certified to teach breast exam technique and provides teaching to all patients needing breast exams and others as requested.
· Provide emotional support and coping strategies to help patients navigate the emotional challenges associated with a breast cancer diagnosis and treatment.

Education, Community and Support Services:
· Provides education on breast self-exams to community groups.
· Develops and presents public education programs to groups as requested and through the Health Center for Women lecture series.
· Writes articles for publication as needed and interfaces with media (newspaper, radio, television) when requested.
· Provides current information regarding breast health, Arnot Ogden Medical Center (AOMC) services and scheduled programs to webmaster for incorporation in AOMC website.
· Coordinates activities for Breast Cancer Awareness month in October of each year.
· Serves as AOMC representative on the Chemung County Breast and Cervical Health Partnership and attends monthly meetings.
· Interfaces with American Cancer Society as needed and serves on the committee for the annual Breast Health Awareness Run/Walk event.
· Participates in health fair and other health related community events as required.
· Manages information flow for all Breast and Cervical Health Partnership clients

Provides cross-coverage for the Breast Cancer Nurse Navigator as needed.
Is responsible for attending all annual mandatory educational programs as required by position.

EDUCATION:
Required: Registered Nurse (RN) with current license in NYS.
Required: Bachelor’s Degree in Nursing
Preferred: Certification in Clinical Breast Examination and Oncology Certification
EXPERIENCE:
Required: Minimum of 5 years in a clinical setting. Well organized and self-directed. High level of proficiency in verbal and written communication.
CARDIOPULMONARY RESUSCITATION (CPR) REQUIREMENTS:
BLS is required.
PHYSICAL DEMANDS:
Appreciable physical effort required. Some lifting, reaching, and stooping.
EXPOSURE CATEGORY:
Category I. Tasks that involve exposure to blood, body fluids or tissue.
