NoPOSITION SUMMARY:The Registered Nurse, Diabetes Educator is a member of the interdisciplinary team working in the out-patient diabetes education program. The RN utilizes the care process consistent with their license or certification including initiation of interventions within individual scope of practice. The RN incorporates diabetes education principles that are current and reflective of the standards set forth by the American Diabetes Association, The American Association of Diabetes Educators and The American College of Endocrinologists in respect to the management of Type 1, Type 2 and gestational diabetes.By participating in educational activities, the RN assumes responsibility for continued professional and personal growthQUALIFICATIONS:EDUCATION: Graduate of an accredited school of nursing required. BSN preferred.LICENSE/CERTIFICATION: Current license as a registered Nurse in the State of New Jersey. Certification from the American Association of Diabetes Educators as a Certified Diabetic Educator (CDE) preferred. BLSHCP required.EXPERIENCE: Minimum 3 years experience in caring for people with diabetes. Excellent verbal and written communication skills, as well as working as a member of a team. Excellent customer service skills.PERFORMANCE EXPECTATIONS:•Provides out-patient group classroom instruction for patients with diabetes, their significant others, and family members, incorporating all ten essential areas of the curriculum as specified by the American Diabetes Association (ADA).•Provides individual instruction in the out-patient setting, again incorporating all ten essential curriculum areas as specified by the ADA.•Performs individual initial assessment of the patient that includes a relevant medical, diabetes and social history.•Provides face to face assessment of patient's diabetes knowledge, self-management skills, and diabetes related behaviors based on the content areas of the National Standards.•Documents an educational plan which includes measurable learning objectives and patient selected behavioral objectives based on the above assessments (2&3).•Collaborates with the other team members of the Diabetes Self-Management Education instructional team to problem solve, share information, and up-date educational tactics.•Assists with the development and evaluation of educational material used in the classroom and individual setting.•Assists with facilitation of Adult Diabetes Support Group.•Collaborates with in-patient CDE to assist patients in obtaining education post discharge.•This position supports organizational goals by providing quality customer service, participating in performance improvement efforts and demonstrating a commitment to team work and cooperation.Apply Here: https://www.click2apply.net/lllPNqiXZz47JTkbxtJjoQPI241142511
