The Licensed Practical Nurse in the ambulatory care Family Medicine setting is responsible for providing competent and comprehensive care to patients and their families throughout the patient's life span. The nursing process is the basis for comprehensive care with specific focus on health education, health maintenance, disease prevention and continuity of care. The nurse actively participates in the multi-disciplinary approach to health care and views the individual and family as an integral part of that team. The LPN supports and implements the philosophy, goals and standards of JFK Medical Center. Active member in a patient centered-team approach in the care of the patient and families. Works collaboratively with all team members in achieving shared goals with a consistent focus on quality care. Participates in quality improvement activities including data analysis, implementing recommendations, evaluating changes and outcomes.

Responsibilities

A day in the life of a Licensed Practical Nurse at Hackensack Meridian includes:

  • Implements and follows through on any physician orders such as medication and vaccine administration, and performs diagnostic tests such as EKG's, NST's, aerosol treatments, etc. Competently and effectively.

  • Provides patients with educational forms and information such as the VIS as required. Sets up for any procedure with all necessary equipment such as PAP smears, biopsies, skin tag removals, etc.

  • Assumes responsibility and function in the role of Triage Nurse, in the Coumadin Clinic and other specific tasks as they arise.

  • Verifies patient ID and escorts patients to an exam room. Collects pertinent health history and other information from the patient, family, and significant others including allergies, age appropriate vital signs, pain assessment, smoking status, and any other related data as required. Documents in the patient record utilizing hospital approved abbreviations and adhering to JFK documentation standards. Utilizes sound nursing skills, critical thinking, and clinical judgement in both routine and emergent patient assessments. Informs any and all members of the health care team, of any issues and problems which could impact the care of the patient.

  • Able to delegate tasks appropriately to other staff members while maintaining professional standards; and patient satisfaction and safety. Participates in community events as a representative of the department and JFK.

  • Assumes responsibility for professional growth by completing all required hospital educational programs. Active participant in the overall resident educational process. Acts as a facilitator in the event we have not met the patient's expectations in order to resolve the issues.

  • Utilizing sound nursing and clinical judgement, assists other members of the health care team in the delivery of patient care within the Patient Centered Medical Home model. Duties may include follow up calls to patients seen in the ER, completion of forms, refilling prescriptions via approved algorithms, responding to calls to Team Nursing and Team Triage within established time frames, and other duties as required. Communicates critical and vital information either through the Huddles or during daily interaction so patient care planning can be effective and complete.

  • Other duties and/or projects as assigned.

Qualifications

Education, Knowledge, Skills and Abilities Required:

  • Graduate of an accredited school of nursing.

  • Ability to work as a member of a team centered approach to patient care.

  • Demonstrates effective communication skills and the ability to successfully complete orientation and all associated competencies.

  • Computer skills required.

Education, Knowledge, Skills and Abilities Preferred:

  • Ambulatory care experience

  • Acute hospital experience

Licenses and Certifications Required:

  • Licensed Practical Nurse.

  • AHA Basic Health Care Life Support HCP Certification.
