Description LSS of WI and Upper MI is currently searching for a Care Coordinator for our YES program in Eau Claire., WI. YES provides wraparound services to at-risk children and their families, focusing on children and families who have struggled with traditional service provision and are either at-risk for out of home placement or who are working toward reunification. YES provides intensive case coordination and individualized community-based services. Services focus on the identified child\'s well-being and progress, but incorporate a family members and supports, acknowledging the important and crucial roles they play in the lives of children and juveniles. This is a full time, 40 hour a week - benefit role eligible role. At Lutheran Social Services of WI and Upper MI, the health and safety of our colleagues and clients is our top priority!
