JOB DESCRIPTION:A LPN promotes the residents' health, wellness and illness prevention through daily nursing and personal care endeavors. A LPN works under the direction of a Registered Nurse and handles a diverse range of tasks, including administering treatment or medications, performing clinical procedures, and documenting the clinical data of patients, among other duties.

RESPONSIBILITIES:

  • Ensures the care and safety of all residents in the nursing unit

  • Ensures that individual care plans are followed

  • Administers and documents medications, treatments, and other nursing services

  • Makes a count of controlled substances on the assigned nursing unit

  • Records current condition of assigned resident in EMR

  • Receives and gives an accurate report of the resident condition at shift change

  • Communicates to the nursing team and medical provider as indicated and required

  • Assists in the orientation of new staff

  • Maintains competency in all the tasks needed by CMS

REQUIREMENTS:

  • Minimum 18 years of age

  • New York State (LPN) Licensed Practical Nurse licensure

  • Strong written and verbal communication skills; able to follow written and verbal instructions

  • Basic computer knowledge, PCC e-MAR experience preferred

  • Physical endurance for frequent activity; ability to lift a minimum of 20 pounds
