Job Description:
Responsible to assist in program development and coordination between providers, staff and patients for a defined population. Acts as a liaison between internal stakeholders and key community partners. In conjunction with internal departments and disciplines, coordinates community outreach programs, including screening and health promotion activities, and education. The position will include direct patient care for orthopedic patients. Will maintain ongoing collaboration with providers, staff, quality, regulatory and reimbursing agencies are as required for optimal care coordination across the continuum. Position requires knowledge and ability to research evidence-based practices analyze and interpret data to make program recommendations and improve population outcomes.

- BSN or ADN degree from an accredited school or college of nursing
Licensure/Certification Qualifications:
Work Experience:
Minimum of 3 years of nursing healthcare experience
Licensure/Certification Requirements:
Current license with the appropriate State Board of Nursing.
Current Basic Cardiac Life Support (BCLS) certification or ability to become certified within 3 months from date of hire. (11331, 11332, 11333 Pre Proc Assessment does not require BCLS certification)
