Job Description:

Is responsible for caring for vulnerable patients in their home or other community settings to provide health promotion, disease management, care transitions, education and other services as needed. Will work alongside emergency medical services, emergency department, and primary care physicians in both the clinic and hospital setting to identify patients who could benefit from community paramedic services. The community paramedic will work under the license of the medical director utilizing established written protocols in conjunction with the patient’s primary care physician and the patient’s established care plan.

Education Qualifications:

Qualifications:

State licensure/certification as a Community Paramedic is required in the State of MN upon hire

Licensure/Certification Qualifications:

CPR Certification

Advanced Cardiovascular Life Support (ACLS) certificate

Must meet driving requirements and criteria acceptable to Essentia Health's insurer (non-patient transport)

State background check must provide clearance to work with vulnerable adults

Current (valid) US/State driver's License
