Catholic Health Initiatives Coordinator RN in Devils Lake, North Dakota
Overview
Monitors and evaluates patient care practices in the acute and swing bed units. Performs quality/utilization/case management functions. Accountable to coordinate a patient’s care across a continuum of care. Facilitates positive clinical and cost effective outcomes. Serves as a resource for the medical staff and nursing. Works well clinically and interpersonally with the Medical Providers.
Provides education to Medical staff and nursing regarding documentation to support admission and diagnosis. Understands standards of coding. Acts as a resource person to promote collaboration and coordination of patient care. Facilitates improvement in the overall quality, completeness, and accuracy of medical record documentation. Obtains appropriate clinical documentation through extensive interaction with physicians, nursing staff, other patient caregivers, and coding staff to ensure that clinical documentation reflects the level of service rendered to patients is complete and accurate.
Responsibilities
Performs chart reviews for appropriate level of care.
Coordinates discharge plans with discharge planning team and Primary Care Physician.
Documents discharge planning as an ongoing review.
Monitor and notify physicians of observation hours and length of stay.
Collects data regarding core measures and provides feedback to Physicians.
Facilitates appropriate clinical documentation to ensure that the level of services and acuity of care are accurately reflected in the medical record. Improves overall quality and completeness of clinical documentation.
Stays current with, and conducts ongoing clinical documentation management program education for new staff, including new clinical documentation specialists, physicians, and nursing and allied health professionals.
Improves the overall quality and completeness of clinical documentation by performing admission reviews for specific patient populations using clinical documentation guidelines.
Assists in the medical screening process by documenting appropriateness of patient admission, working coding information, and computer system as appropriate.
As appropriate, documents, analyzes data and reports instances of inappropriate patient care, discharge delays, etc. to the Patient Care Manager.
Qualifications
ADN or higher
Current North Dakota RN license
3-5 year’s current clinical experience in acute care setting.
Ability to operate computer programs such as Microsoft word, Meditech, Med Assets, and Milliman.
Preferred
- Prior Case management/ Utilization Review experience.
Pay Range
$33.60 - $48.73 /hour
