Henry Ford Health System RN- Postpartum Support Navigator- 40 hours- day shifts in Detroit, Michigan

GENERAL SUMMARY :

  • The postpartum ambulatory nurse navigator is a registered nurse dedicated to serving as the critical point person for Henry Ford Health high-risk patients delivering prematurely. Supporting this vulnerable population for one year while addressing maternal-infant health inequities will drive meaningful change in our community and improve outcomes.

  • Able to work in multiple environment settings, medical office setting with some remote work.

  • Nurse Navigators are nurses with specialty-specific clinical knowledge who offer individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers across the continuum of care. Navigators Collaborate with other health care team members including physicians, nurses, advanced practice providers, social workers, financial counselors, community health workers, patient access coordinators, and ancillary and other support staff, and referring physicians to create a high-quality, efficient, safe, and exceptional patient experience.

    Essential duties and responsibilities:

  • Follow up on outcomes and metrics for all preterm deliveries, ensuring continuous monitoring and support up to one year postpartum.

  • Provide comprehensive education to mothers on key topics such as infant safe sleep practices, vaccination schedules, and breastfeeding.

  • Tailor educational materials and sessions to the specific needs of the preterm population, ensuring understanding and compliance.

  • Ensure that patients attend all scheduled outpatient appointments, including pediatric care and maternal health visits as required.

  • Coordinate with healthcare providers to facilitate seamless care transitions and follow-up.

  • Work closely with community health workers to address social drivers of health and provide holistic support to mothers and infants.

  • Collaborating with system perinatal nurse on data collection and outcomes research.

  • Serve on monthly maternal infant health equity strategic taskforce meetings.

  • Work with team members to plan, develop and implement educational programs and seminars in the community and within Henry Ford Health.

  • Develop/maintain educational tools for patient/family to promote adherence with treatment plan.

  • Participate in community events and initiatives aimed at reducing health disparities and improving outcomes for preterm infants.

    EDUCATION/EXPERIENCE REQUIRED:

  • Graduate from an accredited school of nursing required.

  • Bachelor's Degree preferred.

  • Specialty experience preferred.

  • Maintains and demonstrates current clinical knowledge in area of specialty.

  • Strong organizational, leadership, listening, critical thinking, problem solving, and interpersonal skills.

  • Strong communication skills, written and verbal.

  • Ability to prioritize and reprioritize quickly.

  • Ability to develop collaborative relationships both internally and externally.

  • Ability to work in teams and in a self-directed, autonomous manner.

  • Able to delegate and coordinate efforts of the team/co-workers.

  • Compassionate and empathetic.

  • Demonstrates excellent coaching and mentoring skills.

  • Takes necessary actions to develop and maintain personal job competencies/expertise and to improve performance on an ongoing basis; keeps current regarding medical and technological trends in healthcare overall and/or specialty/service line best practices, as well as a working knowledge of insurance rules and community resources.

  • Computer skills and knowledge of Windows, Outlook and electronic medical record.

    CERTIFICATIONS/LICENSURESREQUIRED:

  • Current licensure by the Michigan State Board of Nursing as a Registered Nurse.

  • Three (3) years clinical obstetric/neonatal nursing experience.
