Henry Ford Health System Practice Transformation Project Specialist in Detroit, Michigan
GENERAL SUMMARY:
The Practice Transformation Project Specialist coordinates high-impact transformation projects and performance reports to ensure successful implementation of workflows, procedures and project goal accomplishment to consistently achieve the Triple Aim goals of improved health outcomes, improved experience of care and improved per capita cost of care. In addition, the Practice Transformation Project Specialist will function as the liaison between post-acute facilities and agencies and the health system.
PRINCIPLE DUTIES AND RESPONSIBILITIES:
Partners with leadership and team members to identify and support standard improvement activities across post-acute providers as appropriate to improve network-wide performance.
Serves as team lead on various transformation projects related to payor initiatives and the triple aim.
Accomplishes project objectives by applying project management tools to collaborative planning, coordination and tracking of project activities.
Gathers and interprets multiple sources of data and information, generate summaries and ensure post-acute providers have accurate information, tools and support for performance improvement activities.
Trains and educates staff, community partners and physician practices on key elements of program requirements, practice obligations and resources, data collection tools and procedures, quality standards and measurement methodologies and interpretation of findings and reports.
Assists in the preparation of proposals, reports, updates, and summaries for all assigned projects within designated timeframes.
Participates in sharing and learning related to improvement initiatives and other similar activities.
Establishes successful working relationships and interacts effectively with colleagues, leadership, committee and board members, providers, practice managers, community partners, management, and other staff to solicit input and buy-in from clinical program customers/end users and staff.
Maintain a working knowledge of other healthcare related initiatives, cooperate and encourage collaboration to share resources, tools and materials whenever possible.
EDUCATION/EXPERIENCE REQUIRED:
Bachelor degree and at least three (3) years of work experience in health related field.
Experience providing project management services for clinical network, provider organization or related entity.
Demonstrated effective verbal and written communications, and interpersonal skills necessary to interact and build strong relationships with physicians, management, staff, external agencies, customers and patients/families.
Proficient computer skills in word processing and spreadsheet software.
Ability to interpret performance data from multiple sources to determine and implement effective action and solutions.
Demonstrated capability to prioritize multiple conflicting duties, and an ability to adapt to diverse customer population.
Previous completion of project management training or within six (6) months of hire.
Must meet or exceed core customer service responsibilities, standards and behaviors as outlined in the HFHS’ Customer Service Policy
Must practice the customer skills as provided through on-going training and in-services.
PHYSICAL DEMANDS/WORKING CONDITIONS:
Requires 80% local travel to post-acute care facilities within the market area.
Unpredictable environment with wide variations in the types of patients and acuity levels, varying conditions regarding temperature control and potential exposure to hazardous materials.
Normal hazards associated with healthcare environment.
