Center for Life Management (CLM) provides the area's most comprehensive array of outpatient mental healthcare. We offer innovative and evidence-based services that enhance the health and well-being of individuals and families who experience mental illness, serious emotional difficulties and/or co-occurring disorders.
Our Medical Team is comprised of compassionate Psychiatrists, Psychiatric Nurse Practitioners, Nurses, Transcranial Magnetic Stimulation Treaters, and Administrative Assistants who work as interdisciplinary team members in our Adult and Children’s Departments. The Licensed Practical Nurse (LPN) will provide office-based nursing services to adult clients. This position is full time, 40 hour per week position.
Major Functional Areas and Description of Duties
Coordinate client care with prescribing medical staff as well as clinicians, case managers and community support counselors to ensure clients receive necessary and appropriate care;
Coordinate client care with primary care physicians, specialists and guardians as directed by prescribing staff;
Respond to fill requests from pharmacies, providers and clients;
Administer intramuscular injections as order by prescribing medical staff;
Complete prior authorizations and associated appeals for medications;
Maintain logs of sample inventory and refrigerator temperature;
Provide cross coverage for ACT Team and Children’s Department as needed;
Provide timely documentation on client interactions and each service delivered;
Serve on appropriate committees, both internal and external, as determined by supervisor;
Function professionally and in a manner that protects the integrity, confidentiality and rights of all patients;
Execute all required services and documentation in accordance with agency policies and federal/state standards;
Represent CLM in a professional and appropriate manner in all settings;
Perform all other duties as assigned by supervisor.
Qualifications and Skills
Graduate of an accredited LPN program required;
Active Licensed Practical Nurse (LPN) in the state of New Hampshire required;
Maintenance of current professional memberships, licenses, accreditations, and certifications required to perform clinical services;
Ability to demonstrate responsibility and accountability to ensure a therapeutic environment;
Ability to read, understand, apply practice standards, technical procedures, government standards and regulations;
Ability to work with confidence and exercise sound clinical judgment and clear, well thought out clinical decisions and actions in routine, urgent, and emergent situations;
Ability to communicate effectively and compassionately with clients and their families;
Ability to work cohesively with other professionals across disciplines, inside and outside of the agency;
Proficient computer skills, specifically with all Microsoft office applications;
Ability to document effectively and efficiently in course of work;
Ability to navigate an electronic health record;
Ability and desire to work as part of a team while also able to be self-directed and motivated with minimal supervision;
Ability to acquire and effectively practice new skills as needed according to evolving agency needs/services;
Effective organizational skills and ability to multi-task.
Physical Demands and Working Conditions
The work takes place in an office setting. Manual dexterity is needed to use computers and standard office equipment. The ability to sit for extended periods of time is required.

The Center for Life Management has partnered with New England College to offer a Master’s Degree! This is an exciting opportunity for CLM staff to earn a Master of Science in Clinical Mental Health Counseling (MS) degree in 3 years.
Who’s Eligible and What’s Required:
Must be a regular, full-time CLM employee in good standing for 9 months or more.
Must hold a Bachelor’s Degree.
Must agree to a five (5) year commitment of full-time employment in good standing with the Center for Life Management (beginning from start date/year of Professional Development Program participation).
Participants must maintain a B average to continue participation in the program.
