Health Center At Franklin Park Director Of Social Services (LSW Or LCSW) in Denver, Colorado
Mountain Vista Senior Living, an American Baptist Homes of the Midwest community, is looking for a knowledgeable and compassionate Social Services Director (LSW or LCSW) to join our team. Lead and be part of something special! Share your passion for supporting residents and a top-notch nursing team and join us at Mountain Vista! Pay from $64,500+ based on experience. This is a full-time day shift position working 8am-430pm. As the Social Services Director you will provide a high level of customer service to residents and family members; conduct SS assessments, and social histories, work with community agencies and update care plans and MDS. Part of your role will be to address resident and family concerns, plan discharges, handle abuse investigations and finalize State reports. Lastly, you will manage a team of social workers.
What you will need: Must have knowledge of Social Security, Medicare and Medicaid. Bachelor's Degree in Human Services or Bachelor's degree in Social Work from an accredited school of social work required. Master's preferred. Licensed Social Worker (LSW) required. LCSW preferred. Social work experience required; long-term care experience required. Supervisory experience required. Who we are: We Care! A lot!
