Summary
The Social Worker II is Social Worker and/or Mental Health Counselor with strong clinical knowledge and leadership skills.  The individual in this position is responsible to facilitate care along a continuum through effective resource coordination to help patients achieve optimal health, access to care and appropriate utilization of resources, balanced with the patient’s resources and right to self-determination.  The individual in this position has overall responsibility for ensuring that care is provided at the appropriate level of care based on medical necessity and to assess the patient for transition needs to promote timely throughput, safe discharge and prevent avoidable readmissions.  This position integrates the national standards for case management scope of services including:

• Utilization Management supporting medical necessity and denial prevention
• Transition Management promoting appropriate length of stay, readmission prevention and patient satisfaction
• Care Coordination by demonstrating  efficient throughput efficiency while assuring care is sequenced and at the and at appropriate level of care 
• Compliance with state and federal regulatory requirements, TJC accreditation standards and Tenet policy
• Education provided to physicians, patients, families and caregivers
• Provides educational programs for Case Management Department on community resources use in discharge planning
• Provides reports to Behavioral Health Director, DCM, and hospital leadership as requested
• Serves as a resource for other case managers in providing guidance when needed.

 
The Social Worker II collaborates with the patient, family, physician, interdisciplinary team, and payer representative to assess, plan, implement, coordinate, monitor and evaluate options and services to promote optimal patient outcomes.  The Social Worker II monitors the provision of cost-effective care to ensure care is given in the most clinically appropriate setting.  The Social Worker II facilitates compliance with regulatory requirements and organizational performance standards.  Responsibilities include the following activities:
 

• accurate medical necessity screening;
• care coordination;
• transition planning assessment and reassessment;
• implementation or oversight of implementation of the transition plan,
• leading and facilitating multi-disciplinary patient care conferences;
• managing concurrent disputes;
• making appropriate referrals to other departments;
• identifying and referring complex patients to Social Work Services;
• communicating with patients and families about the plan of care;
• collaborating with physicians, office staff and ancillary departments;
• assuring patient education is completed to support post-acute needs;
• timely complete and concise documentation in documentation system;
• maintenance of accurate patient demographic and insurance information;
• identification and documentation of  potentially avoidable days;  
• identification and reporting over- and under-utilization; and
• other duties as assigned.

Qualifications

:
Education
Required:  Graduate of an accredited school of Social Work or Mental Health
Preferred:  Master’s Degree in social services field
Experience
Required: 
Three (3) years experience in the relevant clinical specialty (e.g., social workers for pediatrics patients should have pediatric nursing experience); Minimum of one (1) year acute hospital patient care experience required.
Certifications
Required: BLS
Preferred: Licensure
