Claratel Behavioral Health Case Manager - DeKalb Addiction Clinic in Decatur, Georgia

ACCOUNTABILITY Center Director

DUTIES AND RESPONSIBILITIES

  • Serve as the case manager for individuals formerly living in homelessness or requiring supports to maintain housing with low-income with mental illness or co-occurring mental illness and substance abuse disorders
  • Provide independent residential services to a caseload of no more than 35 individuals and/or families

ASSESSMENTS

  • Day to day and weekly assessments of clients in a residential treatment program
  • Meet minimum direct service time of 80 billable hours a month
  • Ongoing assessments of the treatment environment to maintain a safe and effective milieu
  • Incorporate information from ECR to ensure effective implementation of treatment plans
  • Attend clinical team meetings and provide feedback on caseload

COMMUNICATION

  • Acts as a role model to demonstrate appropriate communication skills
  • Provides mediation for clients in conflict situations and seeks professional assistance from supervisor when needed
  • Immediately communicates issues of safety and/or client health changes to supervisor

CLINICAL

  • Provides highest level of services to clients as defined in program standards and service guidelines
  • Assists in providing clinical programming at site
  • Provides high quality documentation and adherence to agency guidelines
  • Maintains ethical standards
  • Supports clinical leadership to promote best practices

COMPLEX CASE MANAGEMENT (DEVELOPMENT)

  • Participates as part of an interdisciplinary treatment team to provide input for client treatment needs based on observation of the client
  • Obtain information from consumer families, reports, chart, and other staff to work with treatment plan implementation

WORKING CONDITIONS

  • This position provides services in congregate and scattered sites within the DeKalb County community
  • The employee may be required to operate an agency vehicle for the transportation of clients
  • Office locations will be at either a Claratel service center or at a location adjacent to or housed within an apartment complex
  • Office setting Monday -Friday 8:15 am to 5:00pm, and after hours on call support (as needed)

SKILL, COMPENTENCY AND EDUCATION AND/OR LICENSURE REQUIREMNTS

  • Graduate of an accredited bachelor's degree program in human services (Social Work, Psychology, Sociology, etc.)
  • Two years experience in residential services (Preferred)

  • Able to interface in a professional and therapeutic manner with consumers and their families
  • Able to communicate effectively with co-workers, internal and external customers and the public

  • Examine and change their behavior when it interferes with working with clients and other collaborative professional relationships.
  • Comprehend another individuals way of life and values and communicate this empathy and support to the client as a basis for a productive professional relationship.
  • Be objective in systematically evaluating clients and their situations.

  • Serve in an appropriate manner all persons in need of assistance, regardless of the persons age, class, race, ethnicity, religious affiliation, gender and its expression, ability, sexual orientation, and value system.
  • Demonstrate the interpersonal skills to relate effectively and sensitively to students, faculty members, staff, clients, and other professionals. Leadership: Social work students need to show initiative in making professional contributions in their practice setting.
