MEMORIAL HOSPITAL OF LAFAYETTE CO Health Information Technician in DARLINGTON, Wisconsin

JOB REQUIREMENTS: Receives inpatient, outpatient and emergency department patient records and scans or enters them into the electronic medical records (EMR) Identifies whether loose material needs to be scanned or entered into the EMR Performs side-by-side comparison of all manually-entered information to ensure accuracy and follows departmental procedure when correcting errors Assists with prepping processes by removing staples, paper clips, dividers, etc. before scanning, as well as assembles the records in the correct order Ensures records are complete and properly stored and/or organized before indexing, filing or archiving Adequately assigns each document to the appropriate patient encounter and document type Monitors the presence of critical data elements, such as patient name and medical records number, on all scanned documents Deficiencies records for completeness, accuracy and compliance Conducts correspondence with physicians as needed regarding missing or untimely documents, record requests, required signatures, etc. Deficiencies records for completeness, accuracy, and compliance Follows Hospital Policies, procedures, applicable laws, regulations, guidelines and professional records relevant to Release of Information (ROI) and all other HIM functions Works assigned work ques including, but not limited to Deficiency Work Ques and Release of Information Work Ques in a timely manner Uses knowledge and skills to teach, coach and/or mentor others in the ROI and/or Scanning process Maintains confidentiality, is courteous to all customers (Patients, families, and all other hospital staff) addresses and follows up on all customer concerns Acts as a liaison for documentation issues between departments Collaborates and communicates with other areas of the department in matters of mutual concern Assists in responding to walk in and phone requests for information when ROI staff is unavailable Communicates effectively by seeking and giving feedback, articulating ideas, listening and sharing information with all stakeholders Participates in a variety of communication medium to stay abreast of current issues within the department and hospital Exhibits an approachable, interactive style in relationships. Seeks and encourages alternative views Acts with integrity and honesty, being willing to trust others and able to engender trust Trains personnel as assigned Recognizes values and builds on differences and commonalities in people Recognizes and reports problems, errors, discrepancies and unusual circumstances and/or information with possible risk factors to HIM Director Reports documentation that may be regarded as unprofessional, frivolous, insulting, inflammatory or inappropriate to HIM Director Operates designated equipment as directed to complete assignments Demonstrates proficiency in use of electronic health record software Demonstrates effective, skillful and efficient use of designated equipment, including software applications.
