Summary:
Provides assistance with patient care activities as directed on the patient home health aide care plan and under the direction of the registered licensed professional staff. Independently assists the patient with activities of daily living.
Responsibilities:
Assists the patient with personal care activities including bathing, oral hygiene, feeding and dressing..
Assists the client with exercises, ambulation, and transfer activities. This includes knowledge in use of assistive devices such as canes, walkers, crutches, and lifts.
Independently assists the patient with meal preparation/cooking and/or feeding. Must have working knowledge of special diets, diet restrictions, and techniques to maintain sanitary kitchen conditions to prevent food-borne illnesses.
Assists the patient with medication prompts to remind the patient to self-administer his/her medications.
Identifies changes in patient condition that require notification to the patient’s registered case manager. Takes vital signs to include temperature, pulse, heart rate, respiratory rate.
Independently provides basic homemaking and laundry assistance for patient in the home.
Follows policy and procedure for assigned patient care activities.
Maintains a safe and orderly work environment in the patients’ homes and a clean, well-stocked work area. This includes maintaining professional boundaries and integrity of patient financial and personal information in patients’ homes, as well as following network/agency infection and prevention policies and procedures in patients’ homes.
Fulfills all compliance responsibilities related to the position.
Performs other duties as assigned.
Other Information:
Required: Certified Nursing Assistant, or any person who has successfully completed a course or courses comprising not less than seventy-five (75) hours of theoretical and clinical instruction in the fundamental skills of nursing in an approved practical nursing or registered nursing education program. Must have working knowledge of how to cook/prepare meals; special diets and diet restrictions; and techniques to maintain sanitary kitchen conditions to prevent food-borne illnesses. Must maintain a professional appearance and professional communication behaviors in patient homes with both the patients and their family members/caregivers. Able to competently take temperatures, heart rates/pulses, respiratory rates and identify changes in patient condition.
Minimum Experience: six months
