DESCRIPTION OF WORK

As an Aging Care Manager 3, you will assess the situations of older adults and develop care plans to eliminate or reduce any risks reported for persons in need of protective services. You will obtain information from consumers and caregivers, assess all available information, investigate sites for clients at risk, make determinations, and develop plans with each consumer for any services they require.

This position is also responsible for initiating procedures for court related intervention and guardianship cases and participates in supervision, scheduled meetings, annual enrichment, and other trainings. Advance your career and make a positive difference in your community with our team!

Work Schedule and Additional Information: Full-time employment Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with a 30-minute lunch. QUALIFICATIONS: Minimum Experience and Training Requirements: One year as an Aging Care Manager 2 or Aging Program Assessor; or One and one-half years of professional human services, law enforcement, or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or An equivalent combination of experience and training which includes six months of professional human services, law enforcement, or nursing experience.

Legal Requirements: You must pass a background investigation. This position falls under the provisions of the Older Adult Protective Services Act. Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance. This position falls under the provisions of the Child Protective Services Law. Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
