We invite you to join us in providing exceptional care as a Registered Nurse (RN) in our Home Healthcare and Hospice program. We’re seeking dedicated nurses to join our dynamic team serving patients in Brainerd, Crosby, Crosslake, and Aitkin.

Position Overview

We are seeking an independent, adaptable RN who thrives in a collaborative environment, champions teamwork, and brings resilience and professionalism to every interaction. If you’re passionate about patient advocacy, critical thinking, and creative problem-solving, this role is for you!

Responsibilities:

  • Provide direct, compassionate patient care in home settings using the nursing process.

  • Collaborate with patients to identify barriers and create personalized care plans.

  • Prevent hospital readmissions through proactive care and education.

  • Complete OASIS assessments and medication reconciliations in an electronic medical record system.

Qualifications

  • Required:

  • Current Minnesota RN license.

  • Ability to obtain American Heart Association BLS certification within 30 days of hire.

  • Valid Minnesota driver’s license.

  • Preferred:

  • Previous experience in Home Care and/or Hospice settings.

    The pay range for this role is $33.66 to $49.65.
