Cuyuna Regional Medical Center * Home Care Nursing Crosby, MN Nursing Full Time, 8-hour Day Shift FTE: 1.0 FTE (80 hours per pay period) Weekend Requirements: No weekends Posted 05/15/2024 Req # 13151 This position is an internal posting for current CRMC employees to apply beginning at 8:00 am on 5/15 until 4:00 pm on 5/21 POSITION SUMMARY This dual role combines office-based responsibilities with direct patient care, providing a unique opportunity for a licensed practical nurse to contribute to both administrative and clinical aspects of healthcare delivery. The incumbent will manage scheduling patient clinical visits, handle data entry and preparation of patient information for admission, engage in phone conversations with patients for scheduling and necessary patient communications, and provide direct patient care, focused assessments and prescribed interventions to the home health and hospice patient under the direction of a case manager. This position offers flexibility in daytime hours, potential to provide a hybrid of remote and office work opportunities, incumbent will work autonomously under LPN scope of practice and the direction of a case manager, collaborating with a team of licensed professionals. POSITION QUALIFICATIONS Education and Experience: Graduate of an accredited school of nursing. Previous experience in home health or hospice, clinic, rehabilitation setting. License/Certificates: LPN License to practice in the State of Minnesota. American Heart Association BLS certification within 30 days of hire. Valid MN Driver's License required. Special Skills and Aptitudes: Proficient and competent in clinical skills. Effective communication with patients, families, and the health care team. Critical thinking and effective decision making. Demonstrate adaptation and flexibility. Commitment to providing high-quality, patient-centered care and promoting positive patient outcomes. Ability to work independently and autonomously while also collaborating effectively within a multidisciplinary team. ESSENTIAL RESPONSIBILITIES Manage data entry and preparation of patient information for admission, including demographic details, insurance information, and medical history. Engage in phone conversations with patients for scheduling, addressing inquiries or requests related to ongoing care. Manage essential physician orders to augment ongoing patient care, ensuring timely communication and documentation. Oversee recertification orders and ensure compliance with regulatory requirements and organizational policies. Provide direct patient care to the home health and hospice patient. Work independently and autonomously, implementing prescribed elements of nursing process, demonstrating critical thinking skills and the ability to determine when physical assessment deviates from expected physical assessment. Collaborate effectively with a team of licensed and non-licensed professionals to ensure comprehensive and coordinated patient care. Utilize remote work opportunities to manage administrative tasks efficiently while maintaining a focus on patient-centered care. Contribute to a positive work environment characterized by staff engagement, teamwork, and a commitment to patient safety and satisfaction. Coordinate and schedule clinical visits to patients' homes, ensuring alignment with patient care plans and organizational protocols. Job duties encountered while providing direct patient care may include perform physical assessment and collection of vital signs, documentation at point of care, performing wound care, administration of influenza vaccine and other prescribed injectable medications, collection of specimens including venipuncture, CoaguChek point of care collection of specimen for INR monitoring, urinary catheterization, provide patient education, promote health, providing emotional support to patient and families.
